PROGRAM FORMS & WAIVERS
Find the program you are registered in.
You only need to fill these forms out ONCE for DE summer camps and only if you have NOT signed through the registration system.
(Horns camps have different forms).

FriYAY is our Friday Afternoon Club run on campus.
Forms must be signed by a parent/guardian if not done so through the registration system. Multiple children can be added to one form.
Science Sizzle clubs are for Gr 4, 5 or 6 students and run AT YOUR SCHOOL.
If you are signing up online, please remember to bring the first page to your school office.
Arconic STEM clubs are run at your child's school for those in Jr. High.

Nope!
To save you time, please include the names of all your children participating in that club on ONE form. For example, if asked for a child's name, please indicate both (e.g. Toni and Billy or Toni Einstien and Billy Einstien).
Note: All FriYAY clubs are the same form no matter what grade your child/children are in.
Nope! DE consent forms are good for the entire program or term.
If you've signed up for FriYAY sessions 1 & 3, you only have to sign forms once.

Once you have signed successfully, forms will be sent directly to the organizers (DE). You do not need to send us your completed forms again, but you may download a copy for your records.
You may be required to print your signed forms to show your school's administrators, but you do not need printed copies to participate in the club.
