research project underway at Chinook Regional Hospital aims to determine if having medical information at the fingertips of student nurses improves their clinical decision-making at patient bedside.

During the 15-month study, which started last May, about 30 fourth-year nursing students from the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Lethbridge will have an application installed on their mobile phones that contains a wealth of medical and nursing information.

Nursing student Tony Slezina says the app has been invaluable in his work with patients.

Researchers are comparing the clinical decision-making of these student nurses with other student nurses who do not have the app. Students without the app would typically leave the bedside and locate information in a medical textbook or, in the case of a medication issue, consult with the hospital pharmacist. All student nurses are under the supervision of registered nurses at the Alberta Health Services (AHS) hospital.

“The ability to make sound clinical decisions is vital to ensure patients receive safe, high-quality health care,” says study lead Dr. Monique Sedgwick, assistant professor in the Faculty of Health Sciences at the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Lethbridge.

“Health care is an information intensive environment. With this study, we’re trying to determine if clinical decision-making improves with immediate access to current, evidence-based information about patient diagnosis, treatment and nursing care.”

Dr. Sedgwick says nurses make clinical decisions every day that affect patient care. Common decisions include whether to change a bedridden patient’s position to prevent bedsores, whether to alert a physician if a patient’s condition is deteriorating, whether to change a patient’s medication, or determining what supports patients will need after leaving the hospital.

A patient who complains about pain post-surgery requires a nurse to make several clinical decisions at bedside, she adds.

“Nurses must assess the kind of pain the patient is experiencing,” says Dr. Sedgwick. “The nurse must take into consideration when the patient last had medications for the pain, whether the medication worked, whether there were side effects and whether the type of pain is typical following that particular type of surgery.

“Using information gathered from textbooks or the mobile app, nurses decide if the medication will be given, if another type of medication is more appropriate, how the medication will be given, how much medication will be given, or if medication is the most effective way to relieve the pain.”

The application used in this study was developed by the U.S.-based medical software firm Pepid, and contains as much information as would be found in multiple textbooks.

Nursing student Tony Slezina says the application has been a huge help during his work placement in the intensive care unit, allowing him to answer questions for patients and their families right at bedside.

“We can look things up on the app and say, ‘OK, yeah, we’re on the right course here,’ ” he says.

Adds Dr. Sedgwick: “Preliminary findings suggest the app helps students in their reasoning and clinical decision-making. That said, no conclusions can be drawn until the study concludes in July 2014.”

Chinook Regional Hospital is a teaching hospital where, each year, more than 600 nurses are trained along with numerous sub-specialties, internships and other clinical placements.

Site manager Ryan Thomson says he’s pleased the facility is contributing to this study.

“I can’t say enough about the partnership we have with the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Lethbridge and the unique opportunity to move research projects like this forward here in our own backyard,” says Thomson. “I am passionate about applied research that helps us grow and learn.”

The $10,000 study is funded by the Western North Western Canadian Association of Schools of Nursing and the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Lethbridge Teaching Fund.

Alberta Health Services is the provincial health authority responsible for planning and delivering health supports and services for more than 3.9 million adults and children living in Alberta. The scholarship serves as a memorial to the contributions of Wendy (MacFarlane) Lipinski, a registered nurse who practised for 18 years in Lethbridge and area. It was established this year by the Alberta Registered Nurses Educational Trust (ARNET), with funding from Executive Links/Nursing Links, a Calgary provider of nursing education.

Executive Links owner Terese Fleming wanted to honour the nursing contributions of Lipinski, a lifelong friend, who had also returned to school as an adult student to take classes in fine arts at the U of L prior to her passing.

"I am a nurse myself and, if I was ever in a position where I would need to receive health care, Wendy was the type of nurse I would want caring for me," says Fleming. She is enrolled in doctoral studies at the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Alberta and has a special interest in nursing in marginalized populations. Prior to joining the U of L, she worked as an RN in remote communities, Vancouver's Downtown East Side and at Lethbridge College.

"It is fitting that such an outstanding educator as Em Pijl Zeiber receive the Wendy Lipinski Memorial Scholarship for Advanced Education," says Faculty of Health Sciences Dean, Dr. Chris Hosgood. "We are dedicated to providing educators with opportunities for professional development and nursing students with an outstanding education, and wholeheartedly support Em's commitment to mobilize students to make a difference in their communities."

"It's an honour to receive this scholarship," says Pijl Zieber. "It will help me continue with my research, which is focused on how well nursing students are prepared for registered nursing practice. The nursing profession can make even greater contributions to the health of Canadians by enhancing community health clinical education and by mobilizing students to make a difference in their communities. do is one thing. Actually being able to do it, quite another.

In fact, many nurses experience that dilemma every day, according to Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Lethbridge researcher Dr. Shannon Spenceley. She is exploring ways to lessen this source of moral stress among nurses in the workplace.

The study, which is funded by the national Alzheimer Society Research Program, is the first in Canada to focus on moral distress in continuing care settings, such as long-term care or supportive living facilities.

A nursing professor in the Faculty of Health Sciences, Spenceley is the principal investigator on a study that is examining moral distress among nurses who care for those with Alzheimer's disease (AD) and other forms of dementia.

The Alzheimer Society of Canada reports that more than 747,000 Canadians are living with AD or other types of dementia – and that figure is expected to more than double in 25 years (by 2038).

In Alberta, there are more than 46,000 people with Alzheimer's or related dementias, and over the next 25 years, that figure is expected to reach more than 100,000. Seventeen per cent of new cases are 'early onset', where people are diagnosed before age 65, and 72 per cent of those are women.

A fatal and progressive disease that destroys brain cells, AD affects one's ability to remember things, make decisions and perform routine activities. Most people living in residential care facilities have some form of dementia, including AD.

Nursing caregivers can find themselves in difficult, and ethically complex, situations when trying to provide the best care for residents with dementia.

"It may be that a nurse who is caring for a confused and agitated resident can either use a prescription drug to reduce the agitation or simply sit with a resident until the resident calms down," says Spenceley. "The nurse knows that while the former option may cause undesirable side effects, the latter one will require extra time. In a situation where staffing is short and there are many competing demands, the nurse may feel that the choice he/she feels is right is simply not possible. Such situations create feelings of moral distress, which has been linked over time to nurse burnout, and to nurses leaving a job or even the profession."

To better understand the triggers and effects of moral distress, and to help residential-care managers address staff recruitment and retention challenges, Spenceley's team will interview and survey nursing caregivers in several facilities across southern Alberta.

"Ultimately, we want to develop strategies to mitigate moral distress," says Spenceley. "By understanding such issues, and working to create solutions, it creates the opportunity to address some of the recruitment and retention issues faced in this sector of care. Finally, anything that can be done to create a healthier and happier workforce caring for this complex population is a step in the right direction in terms of providing high-quality residential care."

The two-year project also involves U of L health sciences co-investigators Dr. Bradley Hagen and Dr. Olu Awosoga, as well as Dr. Barry Hall of the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Calgary Faculty of Social Work (Lethbridge Campus) and Dr. Roland Ikuta, the medical director of Seniors Health, Alberta Health Services (AHS) South Zone. Colin Zeiber, executive director of Seniors Health for Alberta Health Services (AHS) South Zone, is the AHS community partner liaison for the project.

<div Clinic/Heart Failure Network, a regular guest lecturer in Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Lethbridge nursing classes and an advocate for more connected and patient-centric health care. Yet, at heart, her professional scope is unified by a very personal philosophy.

"The thing that has inspired me is the connection with patients and families, and the ability to deliver excellent care in such a way that it will empower people to live better lives," says Penner.

Penner relishes the opportunity to work collaboratively with the u of L's Faculty of Health Sciences.

Clearly Penner has passed that inspiration along to others. This year, her longstanding contributions are being recognized with the 2012 Friends of Health Sciences Award, an annual honour from the Faculty of Health Sciences that recognizes an individual or agency that has made a significant contribution to health education and research at the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Lethbridge.

Originally from Bow Island, Alta., Penner completed her initial nursing education at Medicine Hat College and her bachelor of science in nursing degree at the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Alberta. She has spent most of her 25-year career in southern Alberta. Today, Penner supervises five nurses at the Heart Failure Clinic, as well as leads the relatively new and forward-thinking Heart Failure Network, which she helped to create in 2008.

By encouraging greater collaboration between diverse roles such as case managers, acute care workers and even family doctors, the network integrates health services among these practitioners and increases their awareness of their roles relative to their colleagues, and even more significantly, to the patient.

"When we understand what everyone's role is, the patient in the middle gets better benefits and outcomes," says Penner.

With an estimated 500,000 heart failure patients in Canada and more than 80,000 in Alberta alone, the significance of this work becomes even more apparent. At 10,000 admissions per year, heart failure is one of the most common diagnoses for medical admission to hospital, says Penner.

"It's a huge chronic disease. That's why we need an approach that's far reaching to promote better care, better outcomes and wise utilization of health-care resources."

She adds that greater integration tends to increase patient stability, which cuts costs by reducing the burden on busy emergency rooms and contributes to fewer admissions to acute care facilities.

On top of her leadership in the Heart Failure Network, Penner also shares her expertise with the next generation of nurses, a commitment that began in 2003. Today, she lectures for several faculty members in both nursing and research courses, grounding her talks about heart failure in actual case studies that the young nurses-in-training are likely to encounter early in their careers.

"Lecturing is near and dear to my heart," says Penner. "I love having an influence on promoting clinical excellence and building the future of nursing."

She also helps students gain experience by connecting them with practical work experiences in clinics, and helping with other real-world skills such as cardiac assessment.

While Penner is clearly motivated by factors beyond awards and accolades, she says she is "deeply honoured" to be recognized by the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Lethbridge, noting that her work in the academic setting has helped her realize her own personal potential.

"It is so rewarding to be collaborative, to work with the esteemed professionals at the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ and to experience the infectious vitality of new nurses. It motivates me and inspires me to be better."

The Friends of Health Sciences Award reception is Saturday, Oct. 13 at 2 p.m. in the Markin Hall Atrium.

This story first appeared in the October 2012 issue of the Legend. As such, it has all the infrastructure of what's expected in a small metropolis. The ability to not only deal with but also manage first aid situations is an essential service that has been entrusted to First Aid Co-ordinator Jacquie Whiffen RN (BA '10).

The nursing program alumnus has been in her role for just over a year now and during that time has been working to understand the current first aid landscape with the goal of establishing a program that will standardize and further publicize a co-ordinated program. People have been dealing with first aid situations in their own areas for years but overall co-ordination of all aspects has not been emphasized.

"Departments on campus each have their own way of recording incidents and it makes tracking of first aid issues less cohesive," says Whiffen. "Streamlining the process will aid in communication of key problems and aid in ensuring our overall safety."

Jacquie Whiffen sorts through a campus first aid kit, auditing its contents so that it will be stocked and ready for a possible first aid situation.

Much of her work in the first year has been in terms of an audit, including the creation of a grid that details how many employees are on campus at any given time, day or night. She's also been trying to determine just who on campus has been trained in first aid and whether they need to be recertified. Much of her summer work will consist of finding and charting where all the first aid kits exist on campus, then going through them to see if they are properly stocked.

"One of the main things we'd like to try and do in the fall is to present a first aid awareness campaign to the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ community so everyone is aware of how to respond to and report first aid situations or issues," says Whiffen. "The idea isn't to come in and change everything. If it's working, why fix it? Occupational Health and Safety Code has strict requirements for first aid in any workplace and we strive to both meet and exceed requirements to ensure everyone's well-being."

Whiffen is originally from Tumbler Ridge, B.C. and worked in a variety of administrative support roles, always telling the people around her that one day she would become a nurse. After giving birth to her first daughter, she decided it was time to finally make that dream a reality and she enrolled at Lethbridge College, putting her on the path to completing her nursing degree at the
U of L.

When she saw the opportunity to return to the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ after working for two years as a nurse in a variety of roles, she jumped at the chance.

"I loved being here as a student, and for a nurse building a first-aid program presents a great challenge," she says. "I like to be challenged and this as an opportunity to grow professionally. It is great to be part of a place that people look out for each other and provide support when an individual faces a health concern."

As for first aid crises she faces at the U of L, Whiffen, who is based out of the Health Centre, says she sees everything.

"Nurses at the Health Centre see everything from cuts and bee stings to chronic disease related issues such as seizures or low blood sugar, as well as sports related injuries and slips and falls," she says. "In this position, I am looking forward to working with the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ community to ensure people's first aid needs are met."

GET THE FACTS

• Whiffen is expecting her second child and will go on maternity leave in the fall

• Her parents moved from Tumbler Ridge to Elkford, B.C. some years ago, leading her to look at southern Alberta as a possible location to eventually go to school

• Whiffen appreciated the learning model in the nursing program.

After giving birth to her first daughter, she decided it was time to finally make that dream a reality and she enrolled at Lethbridge College, putting her on the path to completing her nursing degree at the U of L.

· Whiffen is expecting her second child and will go on maternity leave in the fall

· Her parents moved from Tumbler Ridge to Elkford, B.C. some years ago, leading her to look at southern Alberta as a possible location to eventually go to school

· Whiffen appreciated the learning model in the nursing program. Just remember, that in addition to sunscreen, cruise wear and a new pair of flip-flops, you have to think about maintaining your health while on vacation. And if you are considering travelling at Christmas or during spring break, your travel planning must start now!

A good place to start preparing for your trip begins at www.voyage.gc.ca, the Foreign Affairs and International Trade Canada website that gives you a lovely checklist (Before You Leave Canada) of reminders about the variety of concerns you need to address before leaving the country.

In terms of your health needs, plan to take extra copies of your prescriptions and medical certificates, and to bring extra medical supplies and even eyeglasses.

Travel immunizations are important and definitely need attention. Because health concerns are rapidly changing in some areas of the world, the best way to get travel health and immunization information is to book an appointment with the Travel Health Nurse at the Public Health office. While there is a cost for this consultation, this person has the most current information on the immunizations needed in your specific travel area.

At your immunization appointment, the nurse will ask where you are going and, using the latest information available from Center for Disease Control or World Health Organization, may direct you to a variety of injections, vaccinations or medication. Some of these medications will be available through Public Health and the nurse may direct other medications or vaccinations to be administered by your family doctor. The sooner you book your appointment the better as some vaccinations take several months to complete.

Do not show up at a physician's appointment expecting that a family physician will know what you need for a trip to an exotic locale.

As for cost, it is expected that you build the cost of public health travel advice, some medications/immunizations, doctor's visits and other charges into your vacation budget. You will be required to pay for all visits related to travel health (even your physician must charge YOU, not Alberta Health Care for your travel medication/immunization/health advice).

Example of costs you may encounter:

Public Health Travel Advice ($35.00)

Medications/Immunizations from Public Health (medication and costs depend on country and current outbreaks)

Physician visit, if needed for medication that Public Health cannot provide ($50-$150)

But remember, if you are travelling and getting away from minus temperatures for days of sun and sand – it will all be worth it!

Lori Weber is the manager of the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Lethbridge Health Centre

This story first appeared in the December 2011 issue of the Legend. to protecting themselves against influenza or 'flu'. But in the academic world, flu can still be a major problem.

In a 13-week semester for example, taking 5-10 days off can be problematic whether you are a student or employee at the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬. So, what can you do individually and what can we do as a community to prevent the spread of respiratory-based illnesses?

On an individual level, handwashing remains as the number one strategy in diminishing your chances of getting ill. If you haven't had soap and water on your hands at least three times during a typical day at the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬, you haven't washed your hands enough.

When should you wash? You should break out the soap and water after sneezing or coughing, even if you have used a tissue, after toileting and definitely before touching food.

If you haven't cleaned your desk surface within the last few days, or if you are frequently touching your mouth and eyes, you are increasing your risk of illness.

If you do become ill, stay home for the time you are sick. Do not come to the Health Centre when you are ill with a cough/cold/flu. Instead, call us at 403-329-2484 and ask to speak with a nurse who will assess you and assist you with getting appropriate self-care resources or guide you to get medical treatment. Also, consider getting a flu shot in order to avoid illness and missing a chunk of school or work.

On a community level, please think about the people around you. If you have roommates, let them know if you are ill. Everyone in the house should greatly increase cleaning around the bathroom and kitchen areas specifically. As well, clean with a good disinfectant to protect the people around you.

Think about the professors who you are facing. Sneezing spreads droplets toward them at 200 miles per hour – covering your cough with an available Kleenex or even your shirt would be appreciated.

Again, staying home if you are ill protects everyone from your illness!

If you are diagnosed with an illness that can be spread easily, call the Health Centre at 403-329-2484 and ask to speak with a nurse. Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬ of Lethbridge nursing program, it was a case of the past meeting the present to benefit the future.

"We're very pleased to continue in the tradition established by the Galt School of Nursing and are proud to educate the next generation of nursing professionals," says U of L president, Dr. Mike Mahon.

For nearly 70 years (1910 to 1979), the Galt School of Nursing trained more than 1,100 nurses who played a key role in providing quality health care to the residents of southern Alberta. The Galt family first established a fund in 1910 that supported nursing students and helped to purchase equipment for the school of nursing.

"It is remarkable that this fund has carried over for more than 100 years, from the pioneering Galt family to the present day," says Donna Karl, RN, and a 1963 graduate of the Galt School of Nursing.

Galt School of Nursing Alumni enjoyed connecting with today's students.

Administered by the Galt Alumni Association for the past 15 years, it provided scholarships to descendants of Galt alumni, but as applications waned in recent years, the association wanted to put the money to good use.

"When we realized that, due to our alumni's aging population, we were not receiving many applications for scholarships, we knew it was time to move this money to a place where it would be utilized and the Ãâ·Ñ¸£Àû×ÊÔ´ÔÚÏß¿´Æ¬'s nursing program was an obvious choice," says Karl.

While descendants of Galt alumni are still in the terms of reference for the rewarding of scholarships, academic merit will weigh heavily. The fund will be endowed to provide scholarships to entering or continuing Bachelor of Nursing and Bachelor of Nursing After Degree third- and fourth-year students.

"Nursing is all about caring for people and I can see that the students today are carrying on in that tradition," says Karl, who, along with other alumni, toured the new Simulation Health Centre with current students.

"The nursing students that I met today are still all about taking care of people but are far more educated than we were because of the facilities available to them. That lab we toured is wonderful!"

Karl, who works part time at Lethbridge College in the Health Services Office, says the alumni association felt a great responsibility to administer the fund.

"For years we've been managing that money like it was our own – there were about six of us who would gather around my kitchen table and make decisions about scholarships," she says. "We looked after that money very well and gave it out very carefully. I'm happy that the U of L will be able to look after that from here out."

That it will live on in perpetuity and continue a century-old tradition of supporting nursing students is gratifying to Karl.

"I absolutely love this profession, and am really pleased to still be working nearly 50 years after being in school," she says. "Much has changed since 1963 – we certainly didn't have the facilities or services today's students have – but we can continue to ensure today's students are supported throughout their education."

GET THE FACTS

· The U of L first established a School of Nursing in 1980.

· There are 698 students currently enrolled in the nursing
